Notes on Completing a SF 1187 Customized for Chapter 296

1. Box 1—Name of Employee:  Complete.  Remember, Last Name, First Name, Middle Name.

2. Box 2 – Employee I.D. Number:  Complete.  What they are looking for here is the full social security number.  If you are emailing the SF 1187, you have options.  If you are concerned about revealing this PII (Personal Identifying Information) on this form, you can send this information by a separate email or call it in to whoever is processing the SF 1187.

3. Box 3 – Timekeeper Number:  Leave this box blank

4. Box 4 – Home Address: Complete this box.

5. Box 5 – Name of Agency:   All that you need here is NPS-WASO-Your Directorate name

6. Name of Labor Organization:  This box should already have “NTEU Chapter 296” listed

7. Dues Blank:  Do not enter a dollar amount here. Instead, insert your pay system (e.g., GS, WG, etc.), grade, and step.   The Chapter/NPS Labor Relations will enter the dues percentage that goes with the grade and step.  To explain the dues costs to potential members, we give a dollar amount per pay period.  That is easy to understand. What is actually used for dues calculations is a percentage.  That way when there is a cost of living increase for everyone (for example, the dues will be adjusted automatically, instead of having someone make changes manual for everyone. Similarly, this is an easier way to account for differences in locality pay.

8. Date next to NTEU National President’s signature:  Leave this box blank.
